Essential Duties: Excellent communication and interpersonal skills to collaborate with healthcare providers, staff, patients, and stakeholders. Basic understanding of hospital operations and steps needed to create a comprehensive discharge plan. Coordinating and scheduling appointments for patients with multiple healthcare providers to ensure continuity of care. Facilitating communication and information exchange between patients, healthcare providers, and other stakeholders involved in the care process. Monitoring patients' progress and adherence to care plans, providing support and education to promote self-management and preventive care. Conducting home visits or post-discharge to assess patients' living conditions, social support systems, and adherence to treatment plans. Advocating for patients' needs and rights within the healthcare system and community resources. Documenting patient interactions, care plans, and progress notes in electronic health records or care management systems. Participating in interdisciplinary team meetings and case conferences to ensure effective coordination and collaboration among healthcare professionals. Proficiency in healthcare management systems and technology, including electronic health records (EHR) and patient management software. Knowledge of healthcare regulations, accreditation standards, and compliance requirements. Commitment to patient-centered care and continuous quality improvement. Ability to build and maintain relationships with community organizations and stakeholders. Additional duties as assigned Required education and experience: Minimum of 2 years of experience in a community mental health setting preferably with some experience in a hospital, crisis center, SMI clinic or behavioral health home.
